Case Managers and the 3 Rivers Model of Service:

Case management services are provided by qualified caring staff. Case Management services are largely focused on assisting clients as they attempt to navigate the complex world of social services such as Medicaid, S.N.A.P. (food stamps) LIEAP (low income energy assistance program), Child and Family Services, Adult Protective Services, and the legal/justice system (including city, district, and federal courts, mental health court, veterans court). 3 Rivers case managers primarily act as advocates for their clients in all situations. 3 Rivers Case Managers follow the 3 Rivers program which states that Case Managers must always work for their clients first, must always act on behalf of their clients in all matters, and must provide full force advocacy for their clients in all matters. Your 3 Rivers Mental Health Center Case Manager will never try to control your life.
